Insurance Requirements:
- Two insurance documents are required:
- General Certificate of Liability Insurance (ACORD)
- for $1M minimum
- with the following wording:
- Additional Insured & Certificate Holder
- M&M Events, LLC
- PO Box 27252
- Scottsdale, AZ 85255
- The Additional Liability Endorsement to the Policy
- General Certificate of Liability Insurance (ACORD)
These two files must be attached to your vendor application.
